refactor: tools 栏目扁平化，去除子文件夹

- 文档全部移至 tools/ 根目录（docker_install、vscode_exception 等带前缀命名）
- 图片按板块前缀重命名（docker-1.png、prisma-verify1.png 等）避免名称冲突
- 更新文档内图片引用与 sidebar_position 排序
- 更新 tools/index.md 目录链接
